NOTICE OF OFFICE FINANCIAL POLICIES

FINANCIAL POLICIES

APPOINTMENTS:  We are not required to confirm any patient appointments made in our office.  You as the patient/ guardian/ parent are responsible for keeping all appointments and notifying the office no less than 24 hours in advance of any changes that you may need to make.  After 3 consecutive missed / short notice cancelled / broken appointments we will not schedule any future appointments.  All appointments that point forward will be considered a same day appointment.  This means that if we have an appointment available within 24 hours of your phone call we will offer the appointment to you. We may also charge a NON REFUNDABLE reservation fee of $50 for patients that have 3 consecutive missed / short notice cancelled / broken appointments. Failure to pay the reservation fee will result in future appointments being withheld.

 

COPAYS INSURANCE AND DEDUCTIBLES:  All co-payment and deductibles are due at the time of service unless you have made other arrangements prior to your procedure.  We accept Cash, Check, Visa, Master Card, Discover, and Care Credit.

 

ACCEPTED INSURANCE PLANS:

We accept Delta Dental, Blue Cross Blue Shield - Alabama, United Concordia, Guardian, Always Care, Principle (United Concordia), Cigna-PPO (Non Alabama), Aetna, Assurant, Met Life, Active Military - Through United Concordia, and Tricare through United Concordia. Please call our office for additional information.

 

RETURNED  CHECKS:  Should this occur we would no longer accept payment in the form of a check.  You will be charged a reasonable fee for this occurrence, which must be paid immediately along with balance of the check.  We will only accept Cash or Credit Card payments from this point further.

 

FINANCE CHARGES:  You may be charged a finance charge not to exceed 15.99% per month that your balance is over 30 days past due.  This charge will continue to accumulate each month that the balance is not PAID IN FULL.  After 120 days the account is delinquent, we reserve the right to send the account to collections.
